Full Job Description

The role of Central Care Coordinator is pivotal to the smooth running of the PCN providing administrative support and coordination across a broad number of areas including Vaccination Service, Enhanced Health in Care Homes, First Contact Physiotherapy, Home Visiting Service and the administration of Enhanced Access hours and other PCN contractors.

We are looking for a pro-active and organised individual to join our PCN Core Team, to help coordinate the care of patients and access to the wider healthcare team. You will with multi-disciplinary team members from across the PCN and wider network health and social care network.

This is an exciting opportunity to make a difference to patient care.

We welcome applications from candidates with or without direct NHS experience, and would consider 18- 24 hours over 3 or 4 days per week (to include a Friday)., Seasonal Vaccination Programme

· Rota management for PCN Vaccinator Team

· Implement Care Home Vaccination Plan

o Consent forms

o Booking visits

o Organising required supplies of vaccine and consumables

o Data recording for ICB

o Maintain outstanding lists for each Care Home

· Creation of Housebound rounds

o Obtain lists by practice

o Verify vaccination eligibility and call to book in

o Create sensible "rounds"

o Organising required supplies of vaccine and consumables

o Maintain outstanding lists for each Housebound list by practice

· Vaccination record in SystmOne

o Input vaccination record following Care Home/Housebound visit

o Follow up individual queries with respective practice IT lead

· Taskbox management

o Review any tasks in Vaccination taskbox daily and action as required

· Stock Management for Covid Vaccine

o Check stock with each practice and order accordingly

o Coordinate delivery of vaccine to practices according to their schedules

o Maintain cold chain records

· Protocol/Standard Operating Procedures

o Ensure all protocols are sent to practice staff and signed

Updating protocols on central records

Central Administration support

· Care Home Support:

o Attend weekly Care Home rounds as required

o Perform follow up tasks required by Care Home lead coordinator

o Assist with development of Care Plans

· Set up and provide PCN Team central services:

o Practice/GP details

o PCN Team information

o Holiday Planner

o Room Rota/Room bookings

o Assistance organising and setting up team and Board Awaydays

· HR Services for the PCN

o Maintain appraisal calendar and ensure team leads have diarised and records are kept as appropriate

o Maintain Training and Development records with Education and Environment Lead

o Manage Starter and Leaver Checklists

o Work with Team Leads to create Induction Programmes

o Timetable and manage induction programmes

o Liaise with PCN teams to facilitate learning programmes

· Operational Support:

o Assist with coordinating Home Visiting Service diaries, holiday plans and sickness cover

o Assist with Physio workplans, holiday and sickness cover

o Prepare Enhanced Access rotas including phlebotomy collection

· Equipment

o Maintain equipment record for PCN Team

o Order new equipment

o Ensure issue and return of starters and leavers equipment

· Financial:

o Check monthly contract invoices, input data as required and update budgets

o Maintain payment ledger for Enhanced Access contract

In delivering these services, you will:

a. utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care.

b. holistically bring together all of a person's identified care and support needs and explore options to meet these within a care plan based on what matters to the person. Communicate this to other health care professionals in a clear and concise way.

c. help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care, using tools to understand people's level of knowledge, confidence in skills in managing their own health;

d. assist people to access self-management education courses, peer support or interventions that support them to take more control of their health and well being;

e. provide coordination and navigation for people and their carers across health and care services, working closely with the wider healthcare teams.

f. support the coordination and delivery of MDTs within the PCN.

Other responsibilities

· Your first point of contact for general advice and support will be the PCN Manager.

· You will work with sensitive patient information so confidentiality and understanding of GDPR will be required.

· You will also be required to have excellent problem-solving skills, attention to detail, be organised and able to work as part of the team.

· You should be proficient in IT programmes, especially excel, and experience using GP IT systems is desirable.

· You will work across all four surgeries in the PCN and so you should be able to travel to Fordingbridge, Ringwood and Bransgore.

· In addition, you may be asked to carry out any duties as may be reasonably requested by the team.

Person Specification

Qualifications:

Essential

-Educated to GCSE (Maths and English A-C)/NVQ level 2 or equivalent level of qualifications and/or equivalent relevant proven experience

-IT literate

Experience:

Essential

- Commitment and willingness to undertake learning and development courses as required to ensure competency for this role, this will include NHS statutory and mandatory training.

- Understanding of confidentiality and Data Protection Act

- Experience of using MS Office Applications

- Clean driving licence

Desirable

- Experience of using SystmOne/EMIS

- Experience in an administrative role (within the NHS or otherwise)

- Experience in patient or customer facing role

Avon Valley PCN is situated around Ringwood and encompasses four practices. The Practices include The Fordingbridge Surgery in Fordingbridge, The Ringwood Medical Centre and Cornerways Surgeries in Ringwood and Twin Oaks Surgery in Bransgore.

As practices we have been working together for some time and have a good and established relationship. Together we aim to build robust services across our network to improve patient care, and are keen to bring new members into this Primary Care team to achieve these goals.
